To save time during your appointment, you may wish to complete these forms in advance and bring them with you the day of your appointment.
Appointments & Consultations
To schedule an appointment please call our office at (212) 794-0044. Please note a credit card (Visa, MasterCard, or American Express) is required to hold an appointment. We respectfully suggest that you arrive for a scheduled appointment at least 10 minutes early. If you have any questions or concerns, please contact us and we will be happy to assist you.
If you are unable to keep an appointment, we ask that you provide us with at least 48 hours notice. If an appointment is not cancelled or rescheduled within this time frame, the fee for that appointment will be charged to your credit card.
Our New York office provides on-call service for emergencies, 24 hours-a-day, 7 days-a-week. Call (212) 794-0044 and Dr. Thorne will be contacted immediately.
Health Insurance Plans
Our office does not participate directly with any health insurance plans. We do, however, accept all insurance plans with out-of-network benefits except Medicare (See Medicare opt-out information). It is our desire to remove any obstacles which could prevent you from your desired procedure. We will help you file insurance claims with insurance carriers and provide all necessary information to insure timely and complete payment of your claim.
